Student Release Waiver Form

Students ages 10 & up may independently enter and exit the building with proper permission from their parent or legal guardian by filling out this form.

Please keep in mind that we are located in a public building that houses several businesses and we are not responsible for your child until they enter our space or after they leave at the end of class.

NOTE: classroom spaces usually open up 5 minutes before class and there will be no access before that time.

We will not release your child before the end of class unless we have received specific instructions to do so.

Permission for independent entry and exit

Students aged 10 & up may be allowed to enter and exit Total Arts! on their own provided a parent or legal guardian has signed this waiver form.

By signing this form you agree that Total Arts! is not responsible for your student before they arrive in their designated classroom space or once they leave the classroom area to exit the building.

Doors to the classroom spaces open 5 minutes before class begins. Please do not send your child in before that time.

It is expected that students will respect the building and the common areas as they enter and exit. Total Arts! may opt to remove this privilege, in writing, at our discretion based on student behavior, building management concerns, and logistical or safety reasons.

A request for this permission to be cancelled must be made in writing.
